1. Download and complete HOF Form

2. Download and complete Family Member form for each applicant

3. Include a 1inch by 1inch photograph

4. Attach a Passport size photograph

5. Pay $10.00 Cash to cover registration and courier charges as charged by Ejamaat head office at Badri Mahal.

6. Submit the above to Mulla Shabbirbhai Ezzi or Adnan Bhai Rangwala
